Surgery Fees
Ambulatory surgery is less expensive than hospital-based surgery and preferred by most insurance companies. Linden Oaks Surgery Center (LOSC) is recognized by insurance companies and health plans, including HMO, PPO & POS (Point-of-Service). We will file the insurance claim for you. Medicare assignment is accepted by LOSC.
We are constantly working to add new insurance contracts to meet the needs of our patients. Please feel free to contact us to discuss your insurance and billing inquiries. LOSC charges a facility fee for each surgical procedure performed by your surgeon. This fee includes the cost of nursing personnel, medications, supplies and the use of the operating and recovery rooms; specialized operating room monitoring equipment and use of the admitting and discharge rooms. You will receive a separate bill from your surgeon and your anesthesiologist, if one is needed for your procedure.
Our fees are estimated charges based on the procedures that are scheduled. Final charges are based on actual procedures performed.

Workers Compensation
We will need a copy of your insurance cards to properly file your insurance. Please check with your insurance or workers compensation carrier for any pre-certification requirements prior to surgery. Patients with insurance are asked to pay their co-payment and any deductible as a deposit on the day of surgery. LOSC will bill your insurance company as a courtesy, however, any balance due is your responsibility.
If you do not have health insurance, we ask that you be responsible for our facility charge on the day of surgery.
